The ARMHS Golden Thread: A Documentation Checklist for MN Providers

DHS auditors trace every billed unit back to a goal. Here's the documentation chain that survives review — and the most common places it breaks.

The ARMHS Golden Thread: A Documentation Checklist for MN Providers

If you've been through a DHS license review for ARMHS, you know the question that derails most of them: "Show me the goal this billed unit is tied to."

That's the golden thread — the unbroken chain from your client's Diagnostic Assessment, through the Functional Assessment, into specific ITP goals, executed via interventions, observed in progress notes, and ultimately rolled up into a billable unit. When auditors ask, they're asking you to walk that chain in two clicks.

The five links

  1. Diagnostic Assessment (DA). A current, signed DA establishing eligibility for ARMHS.
  2. Functional Assessment (FA). Linked to the DA, identifying functional impairments and skill areas.
  3. Individual Treatment Plan (ITP). Specific, measurable goals with objectives, linked to FA findings.
  4. Progress notes. Each session note tied to one or more ITP goals, with intervention and observed progress.
  5. Billing. Each unit billed must map to a progress note, which maps to an ITP goal, which traces back to the FA and DA.

If any one link is missing or weak, the units it supports are at risk in a recoupment review.

Where it breaks (in our experience)

  • Stale DAs. A DA older than 365 days kills downstream eligibility. We see this most often when a client transfers between agencies.
  • ITP goals that don't reference the FA. "Improve coping skills" is not a goal an auditor can trace. "Reduce anxiety attacks from 5/week to fewer than 1/week using grounding skills identified in the 3/4 FA" is.
  • Progress notes that don't name the goal worked. A note that says "Met with client, discussed week" is unlikely to support the unit.
  • Supervisor co-signs missing. MHRP notes need supervising MHP review per service standards. Routing matters.

A two-minute test

Pick any single billed claim from last month. Can you, in two clicks, get to:

  • The progress note that supports it
  • The ITP goal that note worked
  • The FA finding that goal addresses
  • The DA that establishes eligibility

If yes — your golden thread is intact. If no, fix the broken link before someone else finds it.

How Lumix handles this

In Lumix, the chain is enforced at data-entry time. You can't build an ITP goal that doesn't reference an FA finding. You can't write a progress note without selecting an active ITP goal. You can't submit a shift without at least one progress note. And the billing claim links back to the shift. From the claims dashboard, "trace this claim" walks the chain in one click.
